Insurance

HMO Health Plans:

We are not on the provider panel for any HMO plans.

PPO Health Plans:

Fees are paid when services are rendered.  We will provide you with a standard health care claim form that verifies your payment for our services.  You then will mail this form to your insurance company and they will reimburse you for your payment.
